PERSONAL HISTORY STATEMENT

for

POLICE OFFICER POSITION


Any applicant who is interested in applying for and meets the minimum requirements should submit an Application for Employment and Personal History Statement to the Recruitment Coordinator. Application and Personal History Statement may be hand delivered or mailed to Human Resources Department, City Hall, 205 N. River St., Seguin, Texas 78155. If there are no Vacancies pending, your Application and Personal History Statement will remain on file in Human Resources for one year.

All applicants are required to complete the "Personal History Statement" and "TCLEOSE Employment History Records Release" form.

An applicant who has served in the Military is required to complete the "Request Pertaining to Military Records" form.

Print as many of the "Additional Employment History Pages to PHS" as needed and attach to the Personal History Statement.
